| Qualified participants must: |

• Have high blood pressure and diabetes
•
Be on medications for high blood pressure and diabetes
•
Be 18 to 89 years of age |
| Participants may receive: |

Diet and exercise counseling will be given as well as glucose meter with supplies and instructions with be provided. Financial compensation will be provided for time/travel $500.00. |
