| Qualified participants must: |

• Be 18 years of age or older
•
Have type 2 diabetes
•
Be taking insulin with or without any oral medication |
| Participants will receive: |

Medication, diabetes supplies, physical exams, and lab test at no cost. Compensation will be provided to participants. |
