Purpose: This study aims to explore the feasibility of an educational intervention to strengthen workplace learning systems in 4 VA Community Living Centers (CLCs). 2 sites will participate in the feasibility study and 2 sites in the pilot study. In the feasibility study portion of the project educational interventions will be implemented, using a train-the-trainer approach to dissemination. Feasibility will be assessed based on both quantitative indicators regarding training time and numbers of participants, and qualitative data derived from telephone interviews with key informants at each site. In the pilot study phase of the project, both quantitative and qualitative data will be collected to assess factors that may have influenced the effectiveness of the intervention implementation and understand the impact the training on knowledge, practice, and working relationships had on those that participated.
Study summary: Background: VA nursing home care units (also known as Community Living Centers or CLCs) already enjoy staff education, development, and retention rates that are significantly better than those found in the private sector, in part due to systematic emphasis on training, education, and using more highly educated workers in this setting (e.g. a higher ratio of licensed nurses to nursing assistants than found in the private sector). As such, it is well-positioned to make enhancements to performance through further investments in direct care workers' (DCWs) human capital. What may yet be missing are investments in managers' training and practice, which may be necessary to tap into DCWs' knowledge and which may be NH-specific, and even unit-specific, rather than system-wide. To the extent NHs fail to make related changes in management practices, there may be dissonance between initiatives to increase worker involvement and actual operations that impede such initiatives. Objectives: The objectives of this study are to: 1) conduct a small feasibility study to assess whether the educational intervention, specifically as delivered using a train-the-trainer model, is practical to implement as designed 2) conduct a small pilot study to validate the methodology proposed for assessing outcomes of this educational intervention by testing protocols, data collection instruments, and other aspects of study design in preparation for a larger study. Methods: 4 VA CLCs will be enrolled in this project. 2 sites will participate in the feasibility study and 2 sites in the pilot study. In the feasibility study portion of the project, both educational interventions will be implemented, using a train-the-trainer approach to dissemination. Status: The project team is preparing for the start of the project.
Criteria: Inclusion Criteria: - VA Employees: psychologists, nurse-managers and front-line nursing care staff assigned to CLCs will be included Exclusion Criteria: - Those not working in the CLC, - Those working in the CLC but not involved in the training intervention that is the focus of the study (e.g. EMS personnel, dietary personnel)
