Summary:
NYC based company (MedAvante-ProPhase) that trains health professionals is looking for patients diagnosed with Alzheimer’s Disease or Mild Cognitive Impairment to participate in a videotaped interview along with their primary caregiver.
All interviews will be conducted by a health professional and will be used to train health professionals in the assessment of symptoms associated with Alzheimer's Disease or Cognitive Impairment. Videos will be kept confidential and will only be used for training purposes. If you are chosen to participate, we will do a two hour videotaped interview in an office in midtown Manhattan (three hours total time commitment), and you will be paid for your time at the end of the interview with a gift card. The dates are Monday May 6th or Tuesday May 7th.
Qualified Participants Must:
• Have a diagnosis of Mild Cognitive Impairment or Alzheimer's Disease.

Their condition should not be severe.
•
They must have a primary caregiver who will also participate in the interviews. Caregiver needs to be someone who regularly spends time with and/or cares for the patient.

No other major diagnoses.
•
Both patient and caregiver must meet our basic criteria to be involved and get paid (i.e. ability to sign consent and provide proof of employability - I9)
