Summary:

Remote heart failure monitoring has the potential to reduce hospital admissions and mortality an increase quality of life and health satisfaction. ZOLL has developed a novel radiofreqency based Heart Failure and Arrhythmia Mangement System (uCor system). The uCOr System can record you heart rhythm, breathing pattern, activity, body position, and thoracic fluid index. The primary purpose of this study is to gather information about the measurements the uCor device makes in relation to events related to heart failure.


Qualified Participants Must:

• Subjects are being hospitalized for Heart Failure or recently been discharged from the hospital for acute decompensated heart failure and enrolled in the study within 10 days post-hospital discharge.
• Have had a hospitlalization, emergency room visit, stayed in an observation hospital unit, or had an unplanned clinic visit in the last 6 months.
• Be 21 years or older.


Qualified Participants May Receive:
