Summary:
We are currently looking for participants for patients with Type 2 Diabetes. This clinical trial consists of approximately 10 visits in the over a six-year study period.
Qualified Participants Must:
• Be diagnosed with Type 2 Diabetes
•
Have had either a Heart Attack or Stroke OR have cardiovascular risk such as (1) High Cholesterol (2) High Blood Pressure (3) Tobacco use
Qualified Participants May Receive:
Study related care and medication at no cost. Compensation may be available.
