We are doing this study to learn how effective, tolerable and safe an experimental drug called AZD5718 is in treating participants with proteinuric CKD. An experimental drug means that it is not approved by any health authority including the U.S. Food and Drug Administration (FDA), except for use in research studies like this. Chronic kidney disease is a common health problem that can cause a number of complications and may reduce your life expectancy. A protein called albumin is found in urine in patients with CKD. Treatments that reduce the amount of albumin in urine have improved outcomes for patients with CKD. This study is being conducted to assess whether AZD5718 can reduce the amount of albumin in the urine of participants with proteinuric CKD. The study will also determine the most effective dose of AZD5718. The amount of AZD5718 in the blood will be measured to study the rates of absorption into the bloodstream and clearance from the body.

Qualified Participants Must:

• Be at least 18 years of age
• Have a GFR between 20-75 (lab value)

Qualified Participants May Receive:

- Study-related care such as physical performed by a board certified physician and EKG

- Compensation for time and travel expenses

- Laboratory assessments such as blood work
