To lower the risk of developing heart failure (HF) in patients 18+ with hypertension (HTN), type 2 diabetes (T2DM, and previous cardiac event
Qualified Participants Must:
• 18+ years of age
• Have Hypertension
• Type 2 diabetes
• Established CV disease which includes Coronary Artery Disease (CAD)- MI/Heart attack or 50% stenosis of artery or CABG or PCI or Coronary Artery Bypass, Peripheral Artery Diease (PAD)- Stenting or bypass of limb or limb/foot amputation due to circulatory insuffiency or ankle brachial index of <0.9, Cerebrovascular diease- Ischaemic stroke or prior carotid endartecectomy/stent
• At risk for developing Heartfailure (HF)
