Qualified Participants Must:
• Adults 18 and over

Participants must have Type II Diabetes
•
Have obesity or are overweight
•
Qualified participants will have an A1C greater than 7.0%
Qualified Participants May Receive:

- You will be seen by a team of qualified health professionals.
